ACPMEC Release important updates about PROFEESSIONAL MEDICAL EDUCATIONAL COURSES.A sper official notification With reference to your application for Admission, you or your authorized person (with authority letter given by the candidate) should remain present with the following Original documents with self – attested copies and Rs.15,000/-in cash as token fees.
- S.S.C.(10th) Marksheet
- S.S.C.(10th) Passing (Credit) certificate given by board Or Birth Certificate given by Municipal Corporation.
- H.S.C.(12th) Marksheet of all attempts.
- GUJCET 2014 Marksheet
- School Leaving Certificate (12th.) / Transfer Certificate (12th.)
- Caste certificate(if applicable with xerox copy)
- Exclusion from creamylayer certificate obtained after 1st April 2014(for SEBC candidates only with xerox Copy)
- Evidence of Birth-place(if not shown in S.L.C./T.C.)
- Any other specific certificate / Documents as asked by the committee.
- Certificate issued by Municipal Medical College Ahmedabad/Surat for admission to local quota of concerned institute (if Applicable)
- Passport(if not an Inidan Citizen)
- Authority letter should have photograph of candidate as well as authorized person duely signed by candidate.(format of authority letter is available on web site )
